History

1991

• In order to bring down the very high infant mortality rate in Jackson County, the Community Action Agency forms the Center for Healthy Beginnings to offer prenatal, maternal and infant support services at 817 W. High Street

1994

• A Family Practice provider is added expanding services into Primary Care.
• To reflect this expanded range of services, the name of the clinic is changed to the Center for Family Health.

1995

• The Center becomes an affiliate of Cascades Health System (now Allegiance Health) and merges with TLC Pediatrics, a Foote Hospital Clinic at 720 W. Franklin Street.

1996

• The Center expands Pediatric services and adds Internal Medicine providers.
• Pediatrics and Internal Medicine move into suites in the Foote Health Center building at 1100 E. Michigan Ave.

1997

• The Center is granted the initial designation as a Federally Qualified Health Center Look-a-like (FQHC Look-a-like).
• With this new designation, The Center is no longer an affiliate of Cascades Health. We become an independant organization with no outside control.

1999

• The Center passes a milestone by providing more than 30,000 visits in one calendar year.

2000

• The Center begins using the Health Department’s Mobile Unit to provide health services at the Interfaith Shelter at 414 S. Blackstone Street. Soon after, permanent clinic space is provided inside the shelter.
• The Center is granted the final designation as a Federally funded Federally Qualified Health Center (FQHC).

2001

• Dental services are introduced through at merger with the Jackson Area Dental Clinic. The services are expanded to include adults as well as children.
• Our Board of Directors is expanded from nine to eleven seats to represent the dental community.
• The Center for Family Health celebrates 10 years of health care to the underserved population of Jackson County.
• The Center passes a milestone by serving more than 10,000 patients in one calendar year.

2002

• Pediatrics temporarily moves to 724 W. Franklin early in the year.
• In late Autumn Pediatrics, Internal Medicine and OB/Gyn all move to one site at 2200 Springport Road.
• The Center passes a milestone by providing more than 60,000 visits in one calendar year.

2003

• Administrative offices, except billing, moves from High Street to a suite located at 2298 Springport Road.
• The Center passes a milestone by having more then 100 full time employees.

2004

• The Center receives State approval to become a Title X- Family Planning Clinic
• A van is purchased to provide rides for our patients.
• Late in the year, The Interfaith Clinic moves around the corner to 308 W. Franklin Street.

2005

• The Center opens its long planned School Based Health Center (SBHC), a first for Jackson County. The Northeast Health Center is located in the Northeast Elementary School at 1024 Fleming Ave.
• A full-time therapist is hired to expand our services to include Behavioral Health Care.
• Billing moves to offices at 2282 & 2298 Springport Road.
• The Women’s Health Center opens in the lower level at 2200 Springport Road.
• The Center passes a milestone by serving more then 20,000 patients in one calendar year.
• The Center passes a milestone by providing more than 90,000 visits in one calendar year.

2006

• A second SBHC opens at the Middle School at Parkside at 2400 Fourth Street. A vote was held to let the students name the new health facility. The Teen Health Center at Parkside is temporally located in an unused classroom.
• Interfaith Clinic expands its services to provide primary health care to all area patients. The clinic name is changed to Interfaith Health Center.
• The Center passes a milestone by providing more than 100,000 visits in one calendar year.

2007

• The Center sponsors the 30th annual Groundhog Gallop and holds Step into Health Party
• The Teen Health Center at Parkside moves into a new clinic inside the school. Services are expanded to include Dental Services. A glass-mosaic Eagle thanks our many donors.
• The Center announces new logo to reflect mission of Opening the Door to Health Care for All.
• A full time therapist is added to the Interfaith Health Center, expanding services into behavioral health.

2008

• The Center sponsors the 31th annual Groundhog Gallop and holds Step into Health Party.
• CFH joins Jackson County Medical Records (JCMR) allowing The Center to participate in county-wide electronic medical record access for our patients.

2009

• The Center sponsors the 32nd annual Groundhog Gallop
• CFH annouces plans for a school health center at Jackson High School
• The Center annouces the purchase of the former Art Moehn Car Lot in downtown Jackson and plans for a new $9m health center.

Dental Services

The Center for Family Health Dental Office provides a variety of dental services for children and adults age 18 to 20.

Some prevention and restorative procedures are also availabe for CFH Diabetic and pregnant patients.

Some procedures have a lab fee that must be paid up front. You will be informed of any additional charges that you may be responsible for prior to any procedure.

While the Dental Office offers many cosmetic services, some may not be covered by HealthPay, Medicare or Medicaid or your private insurance. You will be informed of any costs to you prior to the procedure. Certain services require a referral to a specialist in a particular area of dentistry, such as orthodontics.
